Job summary

PAM Health, LLC is seeking a Clinical Navigator to bridge clinical care and business development within our hospital network. The role focuses on building clinician relationships, identifying referral opportunities, and driving patient admissions as part of the care continuum.

The ideal candidate blends clinical knowledge with sales or business development experience and works with diverse internal teams to optimize the referral and admission process while supporting quality patient outcomes.

Qualifications

• Degree in a health-related field preferred
• Licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required
• Prior marketing and/or rehabilitation/LTACH experience preferred

Responsibilities

• Cultivates referral relationships and manages assigned territory
• Performs patient assessments and analyzes referral/admission data
• Collaborates with Admissions, nursing, and staff to facilitate referrals and improve patient satisfaction
