Qualifications

  • Minimum of 3 years of outside sales experience in specialty hospitals, hospice, or similar healthcare setting.
  • Experience with critical care is preferred.

Responsibilities

  • Act as main hub between patient, doctors, social workers, and staff to ensure cohesive care.
  • Determine criteria for LTACH admission and guide care transitions.
  • Explain diagnoses, treatment options, and test results in clear language.

Landmark Hospital of Columbia is adding a Clinical Navigatorto our team. The Clinical Navigator is a Registered Nurse who specializes in guiding patients and their families through the complexities of the healthcare system, particularly during major health events such as a new diagnosis, hospitalization, or a complex treatment plan. They act as a central liaison between patients, medical providers, and community resources to reduce barriers to care, improve patient outcomes, and ensure a seamless care experience. As a Clinical Navigator, you will serve as the primary point of contact for business development, referral management, and community outreach. You will represent our LTACH Specialty Hospital services to facility staff, attending physicians, hospitals, patients, and families, ensuring they understand our compassionate approach to end-of-life care. This role will include public relations, education, and relationship-building between the Landmark Hospital team and the communities we serve.

Core Responsibilities

Clinical Navigators work closely with hospital clinical and case management teams to facilitate referrals appropriate for Long-Term Acute Care

  • Clinical Triage & Monitoring: Reviewing patient symptoms, conducting initial phone screens, and tracking patient progress using Electronic Health Records (EHR)
  • MultidisciplinaryCollaboration: Acting as the main communication hub between the patient, doctors, social workers, and administrative staff to ensure cohesive care as the patient's care needs change
  • Care Connections: Understanding the patient's clinical status and assessment to determine if criteria for LTACH admission have been met
  • Patient Education & Advocacy: Explaining diagnoses, treatment options, test results, and potential side effects in simple, easy-to-understand language, so they are comfortable with the next level of care being recommended
  • Barrier Mitigation: Adapt to evolving business needs and demonstrate professionalism in difficult situations and creatively resolve barriers through knowledge of community resources available to patients
  • System Navigation: Helping patients understand their options, rights, and connecting them to various community agencies and support services.
  • Communication: Exhibit strong communication, negotiation, and public speaking skills
  • Autonomy: Work independently with a self-driven, results-oriented approach
  • Strategic Business Development: Implement and contribute to strategic marketing and outreach initiatives; identify, develop, and maintain relationships with referral sources to drive patient admissions

Qualifications & Requirements

To effectively manage patient care, Clinical Navigators generally possess the following background and skills:

  • Education: Associate's degree, Bachelor’s degree is preferred
  • License: Active Registered Nurse (RN) license
  • Experience:
    • Previous experience in patient advocacy, health education, case management, or a clinical/hospital setting.
    • Minimum of 3 years of successful outside sales experience in specialty hospitals, hospice, or a similar healthcare setting
    • Experience with critical care is preferred
  • Key Skills:
    • High-level problem-solving, strong emotional intelligence, compassionate communication, and in-depth understanding of the inner workings of healthcare systems;
    • Strong written and verbal communication skills, including public speaking and presentations;
    • Entrepreneurial mindset and ability to manage sales territory effectively
    • Strong organizational skills with attention to detail.
    • Technical Proficiencies: Familiarity with Electronic Health Records (EHR) systems and comfort with digital technology platforms.
  • Travel:
    • Daily travel within the assigned territory will require a valid driver's license and reliable transportation (registered and insured).
