Job summary

BAYADA Home Health Care is seeking a Clinical Manager in the Assistive Care/Home Care program to lead clinical oversight for home-based clients in Arizona. This RN role collaborates with the office team to ensure safe, compliant, high-quality care and optimal client outcomes.

The Clinical Manager will assess clients, develop and monitor care plans, and supervise field staff while ensuring regulatory adherence and documentation accuracy.

Qualifications

  • Graduate of an accredited school of nursing; RN license in good standing in the state of Arizona.
  • Minimum of two (2) years of recent clinical experience, preferably in home care, med-surg, LTC, or related setting.
  • Open to new grads and/or newly licensed RN's.

Responsibilities

  • Conduct comprehensive client assessments in the home and complete the care planning process, including initial and ongoing reassessments.
  • Develop, implement, and update individualized care plans reflecting client goals, status, risks, and social determinants.
  • Provide ongoing clinical and case management oversight for an assigned caseload in line with BAYADA standards.

Clinical Manager – Assistive Care/Home Care

Position Summary

The Clinical Manager is a Registered Nurse leader responsible for providing clinical oversight and case management for BAYADA Assistive Care/Home Care clients in the home setting. This role collaborates closely with the Director, Service Supervisor, Client Services Manager, and office team to ensure safe, high-quality, compassionate care while maintaining compliance with BAYADA policies, clinical standards, and regulatory requirements.

The Clinical Manager serves as a clinical resource for field staff, conducts client assessments, develops and oversees individualized care plans, and supports positive clinical outcomes through effective care coordination. In partnership with the Service Supervisor, this role contributes to caregiver support and development, client satisfaction, workforce stability, and the achievement of BAYADA's mission and Assistive Care operational goals.

Key Responsibilities
Clinical Leadership & Client Care
  • Conduct comprehensive client assessments in the home and complete the care planning process, including initial and ongoing reassessments as required by policy and payor regulations.

  • Develop, implement, and update individualized care plans that reflect client goals, functional status, risks, and social determinants of health; ensure alignment between written and electronic care plans.

  • Provide ongoing clinical and case management oversight for an assigned caseload, ensuring services are delivered safely, effectively, and in accordance with BAYADA standards and state/federal regulations.

  • Collaborate with physicians and other health care providers to coordinate care, obtain and document verbal orders, and ensure timely, accurate completion of required clinical documentation and orders.

  • Conduct clinical visits (reassessments and supervisory visits) at required frequencies, documenting changes in condition, risks, and interventions; elevate complex cases to clinical leadership as appropriate.

Caregiver Oversight, Development & Engagement
  • Provide clinical supervision, guidance, and professional development for home health aides and other field employees, including skills assessments, competency evaluations, and ongoing education in both home and office settings. Collaborate with the Service Supervisor to promote employee success, engagement, and quality care delivery.

  • Deliver individualized coaching and feedback during client visits, reinforcing safety protocols, procedures, documentation expectations, and BAYADA policy; document follow‑up actions and performance trends.

  • Identify high‑potential caregivers and collaborate with the office team to connect them with training, scholarship, and growth opportunities (e.g., BAYADA recognition programs, scholarship and career pathways).

  • Foster a culture of engagement and recognition by actively using BAYADA Celebrates, BAYADA Bucks, and other recognition tools to motivate and retain field staff.

Client & Family Experience
  • Build and maintain strong relationships with clients and families through clear, compassionate, and proactive communication, reinforcing BAYADA’s commitment to dignity, independence, and quality of life at home.

  • Collaborate with the Service Supervisor and Client Services Manager to support appropriate caregiver-client matches, address client and employee concerns, and foster positive relationships that contribute to service stability and long‑term retention.

  • Maintain and utilize a community resource list and ancillary service knowledge (e.g., adaptive equipment, social supports) to address needs beyond the care plan and help keep clients safely at home.

Collaboration, Operations & Quality
  • Work closely with the Director, Service Supervisor, Client Services Manager, and office team to coordinate client caseloads, staffing needs, employee assignments, visit schedules, and service delivery priorities that support Assistive Care operational and performance objectives.

  • Participate actively in team meetings (e.g., daily standups, weekly forward/Week‑in‑Review, case conferences) to review high‑risk clients, open referrals, and key clinical and operational prioritities.

  • Use sound clinical and operational judgment to prioritize work, maintain caseload, and support office initiatives and quality improvement activities (e.g., audits, incident follow‑up, risk mitigation planning).

  • Ensure timely, accurate completion of clinical documentation in BAYADA systems (e.g., AlayaCare/Viv) to support compliance, billing, payroll, and reporting requirements.

  • Partner with the Service Supervisor in the ongoing coordination of client and field employee needs, including care planning, caregiver support, service issue resolution, client satisfaction initiatives, and efforts that promote continuity of care and a positive service experience.

Required Qualifications
