Job summary

Jobtailor seeks a Hybrid Clinical Manager to lead home health operations, oversee clinical coordination, and ensure high-quality patient care. This role drives interdisciplinary collaboration and ensures compliance with regulatory standards.

You will guide field clinicians on OASIS documentation, participate in utilization review, and support the Director of Nursing with scheduling and call rotation, shaping the clinical excellence of our home health team.

Qualifications

• Active Registered Nurse (RN) license
• Minimum one year of recent RN experience
• Minimum one year of home health experience
• Thorough knowledge of Medicare Conditions of Participation (CoPs)
• OASIS documentation knowledge
• Experience with HCHB (Homecare Homebase)
• Valid driver's license

• Review new patient referrals for clinical appropriateness and approve/co-sign clinical orders
• Provide guidance to field clinicians regarding OASIS documentation and utilization review
• Partner with physicians, therapists, schedulers, QAPI, and field staff to ensure regulatory compliance and continuity of care
• Assist the Director of Nursing with clinical oversight, scheduling support, and call rotation responsibilities

Requirements
