Careage Home Health is seeking a Clinical Manager (RN) to lead in-person office operations and supervise nursing and caregiving staff within patients' homes. The role supports the Director of Clinical Services and Executive Director through care coordination, compliance, and on-call coverage.

Qualifications

Role Overview

Clinical Manager (RN) at Careage Home Health, providing in-person office leadership for home health services in patients’ residences. The role supports the Director of Clinical Services and Executive Director through supervision, care coordination, compliance, and on-call coverage.

Responsibilities
  • Supervise home health services provided to patients in their place of residence in accordance with physician orders
  • Supervise and develop nursing skills/performance
  • Assist with recruiting, hiring, evaluating, training, and disciplinary processes for caregiver/nursing personnel
  • Orient new patient care personnel
  • Prepare, implement, and maintain policies and procedures meeting Medicare/Medicaid/accrediting bodies and state/local laws
  • Coordinate interdisciplinary team services, patient care, and referrals
  • Organize nursing services and develop safe standards
  • Coordinate home health/private duty services with other community agencies
  • Maintain confidentiality
  • Serve as a resource to employees
  • Communicate effectively with physicians
  • Investigate and report problems related to patient care and/or employee well-being
  • Immediately report accidents/incidents/unusual occurrences to the Director of Clinical Services/Executive Director
  • Attend continuing education and share information with staff
  • Monitor equipment use
  • Assist with the agency’s ongoing QAPI program
  • Ensure development/implementation/updates of individualized patient plans of care
  • Review patient medical records for compliance
  • Assist with patient care conferences/staff meetings and maintain documentation
  • Participate in community education projects
  • Ensure supplies/equipment availability
  • Be available to share on-call and perform nursing functions during staff shortage
  • Make staff/patient assignments and on-call
  • Act as Director of Clinical Services in the Director’s absence
  • Continually assess patient needs
  • Supervise Home Health Aide and caregiver programs for compliance
  • Ensure compliance with Licensed Care and Private Duty policies/procedures
  • Participate in orientation/training/continuing education of home care aides
  • Perform other duties as assigned
Requirements
  • Current State of Washington Registered Nurse (RN) license
  • Preferred: 1 year of home health experience
  • Preferred: 3–5 years of experience (management experience preferred)
  • Willingness to travel up to 25% of total hours worked within King and/or Pierce County
Experience
  • Preferred experience includes 1 year in home health and 3–5 years total experience, with management experience preferred.
Licenses
  • Current State of Washington Registered Nurse (RN) license
