Alternate Solutions Home Health Schedule: Monday - Friday, 8:15 AM - 5:00 PM, one week/month admin on-call for escalations only $10,000.00 sign‑on bonus!

SUMMARY

The Clinical Manager provides oversight of all personnel and patient care services. The Clinical Manager is responsible for monitoring compliance, quality and safe delivery of services by following clinical processes. The Clinical Manager is responsible for any and all practices and duties within the scope of practice as outlined by the state. Provides backup to the Administrator/DON as needed.

KEY RESPONSIBILITIES
  • Ensure quality and safe delivery of home health services.
  • Collaborate with intake, providing daily direction and managing referral volume based on staffing availability.
  • Provide oversight for liaison workflow to ensure timeliness of completion and identify education needs for field staff and liaisons, ensuring workflow needs and staffing skill sets are congruent.
  • Ensure Start of Care compliance.
  • Focus on outcome.
  • Monitor as assigned.
  • Implement quality improvement programs in the field.
  • Participate in all liaison/administrative on‑call rotation and patient staffing as needed to manage after‑hours team, staffing needs and patient‑related issues.
  • Manage the on‑call calendar for internal and field staff.
  • Manage PTO, call offs and daily/weekly productivity reviews to ensure proper staffing coverage to meet the needs of the business in the field and internally.
  • Complete field visits as needed.
  • Participate in managing cost of goods, working with monitoring supply costs, overtime, staffing, bonuses and visits per episode.
  • Collaborate with billing department to address authorization pro‑bono requests and ensure patient contact regarding authorization exceptions.
  • Perform new and ongoing liaison training as defined by the Administrator.
  • Manage staffing budget, at the direction of the Administrator, including overtime and bonus oversight; report budget exceptions to Administrator; perform payroll entry of field staff bonuses; review and approve liaison timecards.
  • Provide Administrator/DON with objective data related to internal and field employee performance evaluations.
  • Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted living and families.
  • Assist with monthly Case Manager, Nurse and Therapy meetings.
  • Review clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
  • Process complaint workflow reporting staffing trends, neglect, abuse or theft to Administrator for resolution and follow‑up.
  • Complete field clinician check‑in reviews timely.
  • Maintain an active RN or PT license in the state of employment.
  • Participate in in‑service program development.
  • Consistently demonstrate core values and maintain a professional appearance as a representative of the company.
  • Understand, implement and practice policies and procedures for agency operation.
  • May act as backup Administrator, as needed.
  • Arrive at assigned location on scheduled work day, work according to designated hours and on‑call as needed in office and field nursing responsibilities.
  • Be knowledgeable with nursing/therapy best practices and the scope of practice for a nurse/therapist as outlined by the state for licensed nursing/therapy personnel.
  • Utilize computer and phone continually to maintain constant contact with field staff, physicians, referral sources, internal staff and other departments.
  • Be knowledgeable of ASHN billing, authorization, intake, clinical review and workflow processes.
  • Perform assigned duties with the use of computer and phone.
  • Complete and submit all required documentation within specified company requirements.
  • Have dexterity and vision to complete documentation on computer.
  • Perform other duties as assigned.
MANAGEMENT RESPONSIBILITIES
  • Responsible for day‑to‑day management of field staff.
  • Interview, hire, train and terminate department personnel.
  • Provide consistent coaching and challenge and opportunity to direct reports.
  • Supervise direct reports with consistency and clarity of expectations.
  • Complete yearly evaluations accurately and in a timely manner.
  • Perform coaching, counseling and discipline as needed.
  • Work with direct reports to ensure employee satisfaction and resolution of issues and concerns.
  • Manage timecards.
QUALIFICATIONS
  • Minimum of three (3) years of experience as an RN or PT in an acute care setting.
  • Strong critical thinking skills with the ability to problem‑solve clinical needs.

  • Apply fact‑based knowledge of technical skills and business understanding.
  • Ability to follow directions and work as a team member.
  • Valid driver’s license and auto insurance.
EDUCATION AND CREDENTIALS
  • Current RN or PT license in the state of employment; RN is strongly preferred.
