A Licensed Practical Nurse or Registered Nurse assists the Clinical Manager/Administrator with the coordination of home health services to patients in their residence, following physician orders to provide high-quality care. The Clinical Manager Assistant is available during operating hours (or appoints a qualified alternate) and participates in all relevant activities, including personnel assignment.

Qualifications:
  • Graduate of an approved nursing program and current licensure in practicing state(s).
  • 3-5 years of nursing experience preferred.
  • 1 year of home health nursing experience preferred.
  • Alignment with the agency's philosophy and goals.
  • Ability to exercise initiative and independent judgment.
  • Ability to collaborate with others to achieve common goals.
Responsibilities:
  • Adhere to agency policies and procedures.
  • Develop standards ensuring safe and effective patient care.
  • Assist in staffing and recruitment processes.
  • Support orientation and training of new staff.
  • Contribute to policy implementation complying with regulations.
  • Support interdisciplinary team coordination.
  • Help organize nursing services for safety and effectiveness.
  • Coordinate with community agencies and participate in research and administrative functions.
  • Model professional standards in conduct and appearance.
  • Maintain patient confidentiality.
  • Ensure regular attendance and punctuality.
  • Serve as a resource for staff and communicate professionally with healthcare providers.
  • Report patient care or staff concerns promptly.
  • Participate in continuing education and monitor equipment use.
  • Assist with quality improvement programs and patient care planning.
  • Review medical records for compliance.
  • Support documentation, staffing, and on-call duties.
  • Perform other duties as assigned, including referral and authorization processes.
Working Environment:

Indoor office work, home visits, and travel required.

Lifting Requirements:

Ability to perform physical activities, including bending, lifting, and standing for extended periods.
