Job summary

Human Touch Home Care is seeking a Clinical Manager (DON) to lead nursing and medical professionals within a Colorado home health agency. This full-time role emphasizes care quality, regulatory compliance, and process improvement to ensure excellent patient outcomes.

The candidate must hold a Colorado RN license, have 3+ years in supervision in home health, and possess Medicare/OASIS experience, with a relevant bachelor’s degree. In-person work schedule Mon-Fri 8:30am-5:00pm.

Qualifications

• RN license in Colorado is required.
• Minimum of 3 years of supervisor/manager experience in Home Health.
• Minimum of 3 years of experience with Medicare and Oasis.
• Bachelor's degree in Nursing or health related field.
• Experience with computers and medical software, eMAR.
• Understanding of Medicare/Medicaid standards.

Responsibilities

• Lead, direct and support a team of Nurses and Medical Professionals in a fast-paced home health agency.
• Uphold the company's mission, vision and core values as part of daily operations.
• Develop, maintain, and implement nursing policies and procedures.
• Ensure compliance with local, state, and federal regulations and company policies.
• Promote and champion employee and patient safety and well-being.
• Monitor/audit staff practices and drive quality improvement initiatives.
• Evaluate the nurse program and staff performance to achieve optimal patient outcomes.

Human Touch Home Care is seeking a Clinical Manager (DON) to function as an exceptional manager responsible for the oversite and direction of care management, quality assurance, and compliance for the home care services.

• Excellent organization skills with the ability to multi-task effectively
