Job summary

You will assess patient needs, coordinate with referral sources, and educate partners on LTACH placements and services.

Qualifications

• Active and unrestricted clinical licensure such as RN, LPN, RT, PT, OT, PTA, COTA, SLP.
• Associate or bachelor’s degree preferred.
• 1-3 years of related Workers Comp case management experience.

Responsibilities

• Drive patient referrals and admissions through market research, networking, and partnerships.
• Support clinical assessments to determine appropriate level of care and advocate for LTACH services.
• Educate referring partners on Landmark Hospitals services and LTACH placements.
• Travel within the Athens market extensively to cultivate durable relationships with referral sources.
