Job summary

Heart to Heart Hospice seeks a Clinical Liaison Registered Nurse to expand our presence in the healthcare sector and work with hospitals to facilitate appropriate hospice referrals.

The role requires nursing expertise, strong communication with clinicians, and a proactive approach to developing new accounts while ensuring Medicare/Medicaid and sponsor reimbursements compliance.

Qualifications

• Graduate of accredited Nursing program (Diploma, ADN or BSN).
• Current State license as a Registered Nurse.
• Valid driver’s license and car insurance.
• Two years of RN experience; hospice or healthcare setting preferred.

Responsibilities

• Develop a Hospice presence in the healthcare community with hospitals and providers.
• Perform comprehensive nursing assessment for hospice admission suitability.
• Educate and consult with medical professionals on hospice programs and reimbursements.
• Establish public relations programs to foster relations with physicians and community agencies.
• Responsible for new business development and assisting with account development when needed.
• Ensure compliance with local, state and federal laws, Medicare regulations and internal policies.

What You Will Do:

• Develop a Hospice presence in the healthcare community with hospitals and health care providers.
• Performs comprehensive nursing assessment and evaluation for appropriateness of hospice admission.
• Develop, educate and consult with medical professionals concerning hospice programs, pain management and Medicare/Medicaid/commercial reimbursements.
• Establish public relations programs for interpretation of the agency's services and to foster good working relations with physicians and community agencies. Responsible for the implementation of specific projects and initiatives as directed.
• Responsible for new business development and assisting Community Education Representatives with new account development when needed.
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
