Job summary

Qualifications

• Education sufficient for licensure.
• Upon hire: Oregon Registered Nurse License.
• Upon hire: National Provider BLS - American Heart Association.
• 1 year full-time experience in clinical role.

Responsibilities

• Build and maintain positive referral relationships to increase Home Infusion referrals.
• Work with discharge planners, facility staff, physicians and others to assess appropriateness of home infusion and facilitate patient care transitions.
• Provide consultation and informational presentations on home infusion therapy and serve as a clinical resource and educator for ordering providers, facility staff, patients and families.
