AccentCare, Inc. is seeking a Hospice Clinical Liaison - RN to join our team in Atlanta, GA, serving patients in the Atlanta area and Gwinnett County.

This full-time role involves inpatient visits, pain and symptom management consulting, and educating providers about hospice services. The position requires a registered nurse with local market experience and knowledge of governmental home health regulations.

Qualifications

• Requires RN license in the operating state.
• Minimum 1 year RN experience in local market with knowledge of home health regulations and Medicare requirements.
• Previous medical sales experience is preferred.

Responsibilities

• Provide community and provider in-service education in assigned territory.
• Maintain daily communication with Admissions Department and Team Directors.
• Develop and implement individualized marketing plans with Hospice Care Consultants.
• Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups.
• Act as a liaison with the AccentCare Hospice Medical Director, Team Physicians, and Hospice Physicians on behalf of providers and patients/families.
• Make inpatient visits at contracted facilities and communicate with Admissions Department, Team Directors and/or Primary Nurse.
• Provide consultation on hospice and palliative care.

Hospice Clinical Liaison - RN

Location: Atlanta, GA and some of Gwinnett County

What You Need to Know

The Clinical Liaison, either a Registered Nurse (RN), or Social Worker, will act as liaison between the contracted hospitals and the hospice team, make inpatient visits to hospice patients in contracted facilities, act as consultant for pain and symptom management, provide in-service education to providers, perform pre-admission evaluations for appropriateness for hospice services, initiates the admission process by educating and informing patient/family of hospice and its services. Also obtain informed consents. Provide assessment, initial plan of care and intervention(s), which optimize patient and family directed comfort and quality of life and coordinates discharge planning.

Be the Best Clinical Liaison RN You Can Be

If you meet these qualifications, we would love to meet you:

• One (1) year of RN experience in local market with a strong knowledge of governmental home health regulations, Medicare requirements, and care plan development techniques
• Previous medical sales experience preferred
• Licensed RN currently licensed as a registered nurse in the state of agency operation
• Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order
