The role of the Clinical Liaison RN is to coordinate and arrange home care services between HS and contracted facilities for home health, home infusion, and home medical equipment associated with a clinical referral, except when there is an onsite HME liaison.

Required Qualifications
- Upon hire: National Provider BLS - American Heart Association
- Upon hire: Must be licensed in states wherever care is delivered.
- 1 year of Full-time experience in clinical role.
Preferred Qualifications
- Bachelor's Degree in Nursing
- Home Health, Hospice or Care management experience.
- Health care industry experience.
- Documented successful sales account medical management experience.
- Epic EMR experience.

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
