PeaceHealth is seeking a RN Clinical Liaison Hospice for a full-time, 1.00 FTE, day shift position. The salary range for this role is $50.14 – $78.34, dependent on factors such as education, experience, and seniority. Relocation assistance and a hiring bonus may be available.

This role involves acting as an interface between healthcare providers and PeaceHealth Hospice, developing referral sources within hospitals, maintaining relationships, delivering exceptional customer service, and assisting with advanced care planning and hospice education.

Responsibilities include:

  • Maintaining and growing referral relationships by serving as an educational resource and patient advocate.
  • Providing care coordination, participating in hospital meetings, and identifying hospice-appropriate patients.
  • Reviewing referrals, communicating with care teams, and ensuring smooth transitions to hospice care.
  • Understanding hospice care levels and responding promptly to hospital staff needs.
  • Meeting with patients and families to explain hospice services and coordinate care transitions.

Qualifications:

  • Graduate from an accredited nursing program; Bachelor's degree preferred.
  • Minimum of 1 year experience in hospice, inpatient, or home health care.
  • Current Washington State RN licensure.
  • Basic Life Support certification.
