Collaborating with physicians, case managers, and leadership, you will ensure timely access to care while upholding our mission and guiding principles.

Qualifications

• Licensed as a Physical or Occupational Therapist.
• Experience in a healthcare setting promoting programs or services.
• Strong clinical assessment and admission decision-making skills.
• Proven ability to build and sustain professional referral relationships.
• Patient-focused mindset with a high level of integrity and follow-through.
• Self-motivated, organized, and accountable for results.

Responsibilities

• Evaluate patients and determine admissions criteria.
• Prepares development plans, conducts calls, and presents to potential referral sources.
• Integrates the hospital’s mission and Guiding Principles into daily practice.
