Job summary

PAM Health Rehabilitation Hospital of Greeley is seeking a ClinicalNavigator to be the face of our hospital in Greeley, Colorado. You will meet patients at their worst times and share the potential success they can reach during rehab, inspiring confidence and engagement in their care and discharge planning.

The role works under the Director of Business Development, cultivating referral relationships, managing a territory, and coordinating with admissions and nursing staff to facilitate referrals

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Works collaboratively with Admissions Manager and staff to facilitate referral conversion and improve patient flow.
• Develops and maintains relationships with stakeholders including physicians, nurses, discharge planners and payers.

Education
