Qualifications

• Must hold current, valid licenses to practice in the state of support (RN/LPN/OT/PT/RT or Social Worker).

Responsibilities

• Educate referral sources and problem-solve to build relationships.
• Obtain referrals, complete LTRAX assessments, and participate in planning and executing the department plan.
• Travel frequently by air and ground transportation as required.
• Communicate effectively with medical and administrative staff, patients, and families.
