Job summary

Evergreen Healthcare Group is seeking a Clinical Liaison to support the Billings and Laurel markets. You’ll connect hospitals, physicians, families, and nursing communities to ensure smooth transitions from acute care to post-acute care, with an emphasis on relationship-building and patient outcomes.

The role requires travel across the territory, clinical background in post-acute care, and a compassionate, self-motivated approach. LPN or RN license preferred and equal opportunity employer.

Qualifications

• Active LPN or RN license preferred.
• Clinical background in skilled nursing or post-acute care preferred.
• Excellent communication and relationship-building skills.
• Willingness to travel regularly across Billings/Laurel markets.
• Ability to work independently across multiple locations.

Responsibilities

• Build and maintain strong relationships with hospitals, physicians, discharge planners, case managers, and referral partners in Billings and Laurel.
• Evaluate referrals and assist with clinical screenings and pre-admission assessments.
• Coordinate admissions for timely transitions into skilled nursing communities.
• Serve as a trusted clinical resource for patients, families, and referral sources.
• Partner with facility leadership and admissions teams to meet census and resident care goals.
• Track referral activity and contribute to strategic business development.
