RVNAhealth is seeking a passionate, results-driven Clinical Liaison to join our Business Development Team. In this vital role, you'll be the bridge between hospitals, skilled nursing facilities, and patients-ensuring smooth transitions from acute care to home through Home Health, Hospice, and Private Duty services.
Your efforts will directly impact patient outcomes and satisfaction while strengthening referral relationships and expanding our reach in the community.
Key Responsibilities
- Assess patient readiness: Conduct initial physical, social, psychological, and financial assessments to determine eligibility and appropriateness for home-based care.
- Collaborate & communicate: Partner with case managers, physicians, and families to develop and coordinate individualized care plans.
- Build strategic relationships: Serve as a knowledgeable, trusted liaison to hospitals, skilled nursing facilities, and referral partners.
- Promote services: Represent RVNAhealth in both one-on-one and group presentations, educating the community on our offerings and guiding families through available options.
- Facilitate smooth transitions: Oversee referral-to-admission processes and help patients access coverage and resources-even when financial obstacles exist.
What We're Looking For
Minimum Qualifications:
- 2+ years of sales experience in an acute care setting; 1+ year sales experience in home care or hospice preferred.
- Familiarity with community resources and entitlement programs.
- Ability to work collaboratively and independently in a fast-paced, mission-driven environment.
- Technologically adept: Comfortable using communication tools, CRMs, databases, and spreadsheets.
- Excellent communicator: Clear, confident, and compassionate verbal and written skills.
- Self-starter: Strong initiative with a professional, empathetic presence.
