Qualifications

Responsibilities

• Builds and maintains referral relationships for hospital admissions.
• Coordinates intake and pre-admission screening process.
• Performs clinical judgment for patient admission appropriateness.
• Communicates rehabilitation options to patients and families.
• Secures patient information relating to resources and benefits.
• Acts as initial contact for case management and discharge planner personnel.
