Job summary

Heart to Heart Hospice seeks a Clinical Liaison Registered Nurse to expand its hospice presence and collaborate with hospitals and care providers in the Sugar Land area. The role requires an active RN license, a valid driver's license with car insurance, and at least two years of RN experience, preferably in hospice or clinical settings.

The position emphasizes education of medical professionals, care program development, and ensuring compliance with Medicare and related regulations.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Valid driver's license and car insurance.
• Two years of experience as an RN, preferred in hospice or healthcare/clinical setting.

Responsibilities

• Develop a Hospice presence in the healthcare community with hospitals and health care providers.
• Performs comprehensive nursing assessment and evaluation for appropriateness of hospice admission.
• Develop, educate and consult with medical professionals concerning hospice programs, pain management and Medicare/Medicaid/commercial reimbursements.
• Establish public relations programs for interpretation of the agency's services and to foster good working relations with physicians and community agencies. Responsible for the implementation of specific projects and initiatives as directed.
• Responsible for new business development and assisting Community Education Representatives with new account development when needed.
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
