What Will You Do in This Role?

As a Clinical Liaison, you will be responsible for the immediate referral response process to achieve performance goals. The clinical liaison will utilize information provided through the electronic medical system and the client relation management system to complete clinical assessments. You will review and evaluate prospective resident referrals based on clinical capabilities, bed availability, integrated eligibility, and sex offender status to provide an immediate response to the referral partner and effectively communicate the determination with the community.

Do You Have What It Takes?

• A passion for working with seniors
• A flexible, energetic, and positive personality
• An active, in-good-standing Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice
• A minimum of two years of experience in a similar role within a LTC/SNF/AL/MC setting
• Clinical liaison and admission coordination experience
• Intermediate to advanced computer skills, including experience with an EMR or HER
• The ability to make independent decisions and handle confidential information
• Patience, tact, and a cheerful disposition
• Enthusiasm, flexibility, and personal integrity
• Ability to work harmoniously with team members
• Strong leadership, organization, and time management skills
• A professional appearance and demeanor
• Excellent written and oral communication skills
