Qualifications

Job description

The Clinical Liaison is responsible for assuring coordination and proper placement of patients with BHG services for MAT and / or SUD services by gathering preadmission information, to assist in communication of information about the patient to all disciplines and oversee the patients transition to BHG services. The Clinical Liaison is an experienced clinician who educates the patients, their families and the facility staff about the services and programs provided by BHG.

Summary of Essential Job Functions

The key responsibilities of the Clinical Liaison include but not limited to :

Duties and Responsibilities

  • Interacts with outside agency personnel on a regular basis to conduct assessments of patients selected by the agency for care and who have been referred to BHG for our SUD services

Will be competent in the following :

  • Basic assessment of patients
  • BHG programs and admission requirements for services
  • Completing clinical documentation including admission paperwork
  • Other targeted services as directed
  • Effectively Communicates timely with the discharge planner / hospital social worker to coordinate needed BHG services for patients that have been referred to BHG
  • Maintains information on BHG patients that have been hospitalized and communicates back to the BHG clinical team with updates
  • Maintains relationships with referral sources and serves / acts as a liaison between outside agencies and BHG.
  • Is knowledgeable of contracts and payor requirements for covering services
  • Effectively communicates with BHG clinic staff, medical team, patients and families
  • Communicates with the patient, family and health care team regarding appropriate services, supplies and discharge coordination
  • Reviews patients medical record per hospital instructions and allowance, to obtain primary and secondary insurance payor information and relays this to the BHG team for insurance verification
  • Notifies Clinical team of discharge in a timely manner
  • Facilitates the coordination and instructions to the patient / and family prior to discharge
  • Effectively communicates with the Regional Outreach Manager and Regional Director regarding opportunities and challenges that will contribute to the growth of the program and patients being discharged from the hospital or transferred via the ED to BHG.
  • Maintains confidentiality of patient and proprietary information. Observe legal and ethical guidelines for safeguarding the confidentiality of patient and BHG information. Meets with the ROM and RD routinely to review progress with the accounts
  • Maintains a professional image
  • Dresses in attire that conveys a professional image adhering to BHG dress code policy. Consistently maintains personal self-control and professional decorum in the clinics, hospitals and community.
  • Participates and attends required in-services
  • Demonstrates current knowledge regarding the care and management of patients in OTP / MAT programs and at BHG. Completes all BHG educational modules assigned. Stays current on all new programs and opportunities for patient and hospital support.
  • Contributes to the achievement of BHG mission and goals through effectively participating in committees and / or informal work groups as assigned.
  • Maintains compliance with all accreditations, regulatory, legal and safety requirements.

Qualifications

  • Must have one of the following counselor licensures that are required in the state of the Colorado to support complete patient education and admission assessment into BHG services

LPC (Licensed Professional Counselor)

  • LCSW (Licensed Clinical Social Worker)
  • LMSW (License Master Social Worker)
  • Possesses the ability to deal tactfully with personnel, referral sources, patients, family members, government agencies, and the general public
  • Exhibits efficient problem-solving skills.
  • Organized and works well independently and in groups
  • Enthusiastic, highly motivated personality, self-starter, genuine and empathetic
  • Knowledgeable regarding the state regulatory guidelines and payor guidelines as well as BHG policies and procedures
  • Valid and current driver's license and evidence of automobile insurance
  • Ability to handle stress and meet deadlines
  • Ability to travel to multiple job sites and attend required meetings

  • Ability to demonstrate sufficient knowledge and understanding of human behavior to show potential for rehabilitating drug dependent patients
  • Must have sufficient knowledge of the substance abuse population, treatment planning and the field of human social services to work toward the rehabilitation of the patient community
