Job summary

Andwell Health Partners in Lewiston, ME is seeking a licensed LCSW to provide clinical leadership for our Community Care Team (CCT) and H.O.M.E. programs. You will supervise clinical assessments, care planning, and regulatory compliance while supporting a multidisciplinary team.

You will maintain supervision of staff, review assessments and care plans, ensure HIPAA compliance, and contribute to quality improvement in a mission-driven, patient-centered environment with hybrid work options.

Qualifications

• Master's degree in Social Work, Counseling, or related field.
• Current Maine licensure as an LCSW.
• Strong leadership, clinical assessment, and care coordination skills.

Responsibilities

• Supervise, mentor, and support case management staff.
• Review and sign off on Assessments and Care plans completed by case manager.
• Review and approve H.O.M.E. patient care plans.
• Audit EHR documentation to ensure quality and compliance.
• Coordinate care with providers, community partners, and families.
• Support onboarding, training, and clinical consultation.
• Maintain a small caseload and provide coverage as needed.
• Ensure compliance with HIPAA, MaineCare, and organizational standards.
