Concord Hospital in Concord, NH seeks an experienced Registered Nurse (RN) II to assess, plan, implement and evaluate nursing care for a diverse patient population with multi-system needs in a safe, therapeutic environment.

You will collaborate with the health care team, document care, administer medications, supervise staff when needed, and educate patients and families about the plan of care and discharge preparation.

Qualifications

• NH RN licensure required or eligible.
• BSN preferred and one year of clinical nursing experience.
• Ability to follow policies and coordinate patient care with the multidisciplinary team.

Responsabilités

• Gathers data through history taking and physical assessment and interprets data to plan and implement interventions.
• Develops and revises patient care plans and nursing actions as needs change.
• Documents care provided and patient progress toward outcomes.
• Directs and supervises others in assigned nursing activities.
• Teaches patients and families about the plan of care and discharge preparation.
• Manages time and resources to deliver patient care.
• Administers medications and provides direct patient care.
• Leads the health care team and serves as a resource on shift.
• Precepts new employees and performs other duties as assigned.
