Job summary

Strive Health is seeking a Clinical Lead to guide care for patients with CHF, CKD/ESKD and other chronic conditions. You will work across home visits, clinics, and telehealth, using advanced analytics and interdisciplinary collaboration to deliver longitudinal, personalized care.

The role emphasizes high-touch clinical expertise, patient education, and navigational support for a defined panel, with a focus on reducing hospitalizations and improving outcomes.

Qualifications

• Master's degree in Nursing or similar field.
• 2+ years experience as a Family Nurse Practitioner (NP) or Primary Care NP with cardiology (CHF) or nephrology (CKD).
• 2+ years experience with Electronic Health Records (EHR).
• Current state licensure.
• National Board Certification.
• Current DEA license or eligible within 90 days of hire.
• Current BLS or CPR Certification.
• Active driver’s license with ability to travel regionally.
• Basic computer and Microsoft Office skills.

Responsibilities

• Manages a defined patient panel with accountability for quality, cost and outcomes.
• Obtains patient history, performs exams, orders tests, and formulates a care plan.
• Ensures CKD/ESKD and heart failure care; supports transitions to renal therapies.
• Accurately documents diagnoses to support risk adjustment and care gaps.
• Presents cases and provides clinical support for rounds and meetings.
• Provides in-person and telehealth visits across multiple settings.
