You will provide longitudinal, home- and telehealth-based care with autonomy to tailor treatment plans, collaborate with specialists, and optimize outcomes.

Qualifications

Master’s degree in Nursing or related field.
2+ years experience as a Family Nurse Practitioner (NP) or Primary Care NP with specialization in cardiology (CHF) or Nephrology (CKD).
2+ years experience with Electronic Health Records (EHR).
Current state licensure and national board certification.

Responsibilities

Manage a defined patient panel with accountability for quality and outcomes.
Conduct patient histories, exams, and plan longitudinal care.
Support CKD/ESKD and heart failure management across care settings.
Document diagnoses for risk adjustment and care gaps.
