Job summary

South Sound Behavioral Hospital in Lacey, WA is seeking an Intake Assessment Clinician to screen for medical and behavioral emergencies and determine appropriate treatment placement. You will conduct comprehensive assessments and collaborate with the clinical team to support patients and families throughout the intake process.

You’ll work with our on-site team to coordinate admissions, document findings, and provide compassionate care while guiding patients toward the right level of care.

Qualifications

• Master’s Degree in a human relations field is required.
• Licensed to practice in the State of Washington as a mental health professional (Social Worker, Counselor, or RN with MHP).
• Minimum two years’ post Master’s Degree experience in psychiatric settings including customer contact.

Responsibilities

• Receive inquiry calls and assist callers with scheduling face-to-face assessments or triage to the most appropriate community resource.
• Screen for medical and behavioral emergency conditions through a comprehensive assessment to determine the most appropriate level of care for mental health, substance abuse, or dual diagnosis needs.
• Coordinate with the on-call provider/clinical team to ensure the most appropriate placement at the facility or via referral to community resources.
• Work with the patient and support system throughout the assessment process to alleviate concerns for care and treatment recommendations.
• Complete admission paperwork following hospital policy.
• Fulfill administrative duties to ensure assessment and admission processes are thorough and complete.
• Document and process inquiry calls according to Intake policies.
• Assess and stabilize patients before discharge or coordinate hospital admission arrangements.
• Maintain an intake log with all necessary information documented.
• Make follow-up calls to patients who fail to show up for appointments.
