Job summary

Catholic Health Services seeks a Clinical Intake Case Manager to perform frontline clinical assessment, determining eligibility and facilitating placement in appropriate care levels. This role requires collaboration with physicians and referral sources to ensure timely admissions and seamless transitions.

The ideal candidate has Florida RN (or LPN/Therapy with extensive experience) and knowledge of Medicare/Medicaid processes, with strong customer service and communication skills.

Qualifications

• Active RN license to practice in Florida; 3–5 years in health care facility.
• Or license such as LPN/Therapy with 10+ years in health care facility.
• Active Nurse Practitioner (APRN) license in Florida preferred.
• Experience in Marketing and Admissions.
• Knowledge of Medicare, Medicaid, Managed Care, and other private insurance.
• Understand and navigate managed care processes.
• Ability to work in a fast-paced environment.
• Excellent customer service and communication skills.
• Valid Florida driver’s license.
• Multilingual preferred.
• Knowledge of computer office/clinical software.
• Must read, write and understand English.

Responsibilities

• Perform comprehensive nursing reviews for referrals to assess clinical appropriateness and regulatory compliance.
• Establish and maintain relationships with providers, referral sources, physicians, health care facilities, and the community.
• Discuss cases with physicians and clarify physician orders.
• Educate physicians, case managers, and insurers on facility programs across the continuum of services.
• Coordinate inpatient admissions and the referral-to-admission process.
• Ensure referrals are entered timely and complete prior to admission.
• Coordinate care with referral sources to ensure seamless transitions to post-acute care.
• Serve as primary contact between participants and the CHS team.
• Communicate relevant information and obtain/upload discharge documents for clinicians.
