Qualifications

• BS Pharmacy with equivalent clinical experience.
• Doctor of Pharmacy (PharmD) preferred.
• ASHP-accredited PGY-1 residency desirable.
• One or more years of hospital pharmacy experience preferred.
• Iowa Pharmacist license in good standing or eligible for licensure; must obtain license within six months.

• Assures compliance with HIPAA, DNV, OSHA and other state and federal regulations guiding healthcare
• Adheres to policies and procedures of UPH-DM
• Prioritizes the importance of preventing errors, accidents, and infections within the workplace and reports potential or actual concerns immediately

Education

BS Pharmacy with equivalent clinical experience
