Job summary

UnityPoint Health’s St Luke's Hospital Pharmacy in Sioux City, IA seeks a dedicated Pharmacist for full-time Day/evening coverage, including every 3rd weekend and 1:30 PM–10:00 PM shifts. You will oversee verbal handoffs, verify orders, monitor for drug interactions, and support clinical interventions to optimize patient outcomes across our health system.

Overview

UnityPoint Health-St Luke's Hospital Pharmacy

Full Time

Mostly Days, Every 3rd wknd, and 4 (1:30P-10P) shifts per month

Provides services including but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Works directly with physicians, other health professionals and patients to provide all aspects of patient care, including clinical and distributive tasks, for patients served within our health system. Helps patients and healthcare professionals achieve clinical outcomes and monitor progress toward common goals. Supervises other pharmacy staff in the absence of formal departmental leadership. Performs assignments and projects as directed by the Pharmacy Leadership Team.

Responsibilities

• Verifies all orders generated on shift, keeps work flow up to date and current, and cleans up old orders as soon as possible.
• Completes verbal hand off communication to next shift, to ensure continuity of quality patient care and follow through in interventions.
• Screens and affirms appropriateness of orders including allergy check, formulary compliance, pregnancy & lactation status, dosing, duplicate orders etc.

• Documents adverse drugs reactions & medication errors in the RL system as they occur throughout the medical center.
• Fields and investigates drug information questions. Uses resources appropriately and colleagues as needed to ensure timely and accurate responses. Documents as necessary.
• Checks and assures high quality of IV Room products every time. Ensures timeliness of delivery of final product, utilizing means of maximum efficiency.
• Enters TPN orders into compounder computer system accurately and timely.
• Maintains good rapport with peers, nursing staff, physicians and other departments.
• Becomes involved with Performance Improvement Projects, Clinical Initiatives, and other work projects as assigned, and completes them in a timely manner with attention to outcome. Uses data, good communication tools, and multidisciplinary approach as strategy.

Qualifications

Education:

• Graduate of accredited College of Pharmacy

Experience:

• Prefer one (1) year of hospital clinical experience, Residency

License(s)/Certification(s):

• Current licensure in good standing to practice as a Registered Pharmacist in Iowa or licensure eligible in Iowa.
• Valid driver's license when driving any vehicle for work-related reasons.
• Completes at least 2 hours of stroke education CE annually.
