The Clinical Educator & Home Health RN Case Manager serves a dual role, supporting both clinical staff development and direct patient care. As Clinical Educator, this position designs, coordinates, and delivers orientation and ongoing education programs to ensure competency and compliance across the Home Health and Hospice departments. As a Home Health RN Case Manager, this role also provides skilled nursing services and manages a caseload of patients, ensuring high-quality care in accordance with agency standards, Medicare Conditions of Participation, and Pennsylvania licensure laws.

Primary Responsibilities

Clinical Education

  • Design, deliver, and evaluate clinical education programs to keep staff updated on the latest best practices and advancements in home health and hospice care.
  • Ensure all staff are knowledgeable about current regulations and compliance requirements.
  • Evaluate staff competency and provide continuous education on policies, procedures, and clinical skills.
  • Collaborate with clinical preceptors and leadership to monitor and adjust orientation schedules.
  • Create and implement an annual education plan based on skill assessments and regulatory changes.
  • Provide ongoing training through in-services and ensure compliance with Medicare and agency requirements.
  • Update and track MedBridge assignments; follow up on non-compliance.
  • Assist with documentation training and troubleshooting within the EHR system.
  • Serve as a resource on regulatory updates, participate in QAPI efforts, and support community education.
  • Participate in administrative on-call rotation and provide leadership coverage as needed.

Home Health Case Management

  • Perform comprehensive assessments and develop individualized care plans for assigned patients.
  • Deliver skilled nursing interventions and evaluate outcomes.
  • Collaborate with physicians, therapists, aides, and families to ensure continuity of care.
  • Supervise Home Health Aides per Medicare requirements.
  • Document all care accurately and in a timely manner.
  • Educate patients and caregivers on disease processes, medications, and self-care techniques.
  • Attend and contribute to case conferences and interdisciplinary team meetings.
  • Meet productivity expectations based on patient acuity.
  • Participate in on-call rotation and respond to patient needs after hours.

Qualifications

  • Recommend BSN or ASN with a minimum of 5 years' experience in Home Health or Hospice
  • Previous experience in clinical education, staff development, or related roles preferred.
  • Knowledge of Home Health and Hospice regulations, including OASIS and HIS documentation.
  • Proficiency in Microsoft Office Suite and EHR systems.
  • Strong organizational, communication, and interpersonal skills.
