Job summary

Stamford Health is seeking a Full-time Document Improvement Specialist to join the Case Management Team at One Hospital Plaza, Stamford, CT. The role focuses on clinical documentation improvement, IPPS/JCAHO compliance, and DRG optimization, with EPIC EMR experience required.

The position offers daytime hours Mon-Fri with rotating weekends, comprehensive benefits, and opportunities for professional development within a supportive health system.

Qualifications

• Connecticut Licensed Registered Nurse.
• Bachelor’s Degree in a related field is preferred.
• CCS, CCA, CPC-H certification preferred.
• MUST have EPIC EMR system experience.
• A minimum of 5 Years of experience in case management, managed care, coding and/or case mix - documentation improvement experience.
• Ability to compile objective and measurable data and produce written reports.
• Ability to develop and communicate written policies and procedures.
• Prior training experience and the competencies to develop and present education programs to all levels of healthcare professionals.
• High level of competency with MS Office or equivalent; able to perform data and statistical analysis using Excel.
• Prior Hospital / Health Care experience is required.
• Experience in a Health Care System is highly preferred.

Responsibilities

• Facilitate internal process changes to address clinical documentation opportunities for improvement.
• Improve physician documentation compliance under IPPS and JCAHO requirements.
• Perform concurrent review of Medicare in-patients charts and assess documentation impact on DRG.
• Monitor financial risk and status of Medicare patients on a concurrent basis.
• Assist with compliance and abstraction of JCAHO Core Measures.
• Develop in-service programs with physicians, case management staff and coding manager.
• Provide ongoing education to case management staff and interdisciplinary team.

Education

Registered Nurse license (Connecticut)

Bachelor’s degree in a related field (preferred)

CCS, CCA, CPC-H certification (preferred)
