Job summary

Ohio Living seeks a Clinical Reviewer to ensure accurate patient information for Start of Care and Hospice processes, coordinating with field staff to maintain quality care and regulatory compliance.

Responsibilities include reviewing OASIS, 485s, ROC and discharge data, performing ICD-10 coding, and supporting staff through OASIS training. A COS-C/OCCB certification is preferred, with Ohio LPN licensure encouraged.

Qualifications

• High school diploma or equivalent required.
• Two years post-secondary education in a clinical specialist or medical records program or COS-C designation from OCCB.
• OASIS Certificate and Competency Board (OCCB) certification required or willing to obtain within six months.
• Current unencumbered Ohio LPN license preferred.
• Experience with ICD-10 coding is required.

Responsibilities

• Process Start of Care (SOC) or Hospice Admission Plans of Care timely with field staff.
• Review evaluation packets, OASIS-C, 485s and related clinical information for accuracy and consistency.
• Code ICD-10 diagnoses per standards and ensure accurate service utilization.
• Review ROC and discharge data, and manage documentation for transitions and billing.
• Assist with orientation and training for clinical staff on OASIS documentation.
