Job summary

Ohio Living seeks a Clinical Reviewer to oversee OASIS documentation, ICD-10 coding, and clinical data accuracy for home health and hospice services. The role collaborates with field staff to ensure compliant plans of care and accurate assessments.

Responsibilities include reviewing soc/roc/discharge data, ICD-10 coding, and training staff on documentation standards, with strong emphasis on regulatory adherence and patient care quality.

Qualifications

• High school diploma or equivalent required.
• Two years post-secondary education in a clinical specialist or medical records program or COS-C designation from OCCB.
• Current unencumbered license in Ohio to practice as an LPN preferred.
• Certification in ICD-10 diagnosis coding required or willing to obtain within six months of hire.

Responsibilities

• Process SOC and Hospice plans of care in a timely manner with field offices.
• Review evaluation packets, 485s, and other clinical data for consistency with assessments.
• Verify data accuracy and follow up on documentation corrections.
• Code ICD-10 based on clinical documentation per standards and regulations.
• Ensure care type and Plan of Care align with patient condition and assessments.
• Assist with orientation and training of new Clinical Reviewers as requested.
