This role emphasizes timely patient assessments, multidisciplinary collaboration, and ongoing performance tracking to improve outcomes. The position supports inpatient and outpatient programs with a strong focus on rights, confidentiality, and quality care.

Qualifikationen

• Masters Degree required in a relevant field.
• Licensed clinical social worker or licensed mental health professional required.
• Licenses: LCSW, LPC/LMHC, LMFT, or LMSW/RCSW-I depending on specialty.

• Assures patient assessments are completed in a timely manner by the department.
• Provides direct clinical oversight of the patient access process.
• Works collaboratively with hospital departments to provide services needed.
• Models and trains other behavioral health team members on managing and tracking inquiry calls.
• Tracks and reports on all key indicators related to calls, assessment, admissions.
• Ensures patient rights and confidentiality are maintained.
• Provides input, suggestions and support appropriately.
• Participates as an active member of the multidisciplinary team.
• Responsible for reporting department performance and for identifying trends/patterns that require intervention.
• Completes additional duties as assigned.

Job Summary and Qualifications

May perform patient assessments as needed.

• Assures patient assessments are completed in a timely manner by the department

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
• Masters Degree
• (LCSW) Clinical Social Worker, or (LPC/LMHC) Licensed Professional/Mental Health Counselor, or (LMFT) Licensed Marriage and Family Therapist, or (LMSW/RCSW-I) Lic Master Social Worker
