Job summary

Northern Rivers Family of Services seeks a Director of Community Transition Time Program (CTTP) to lead a new youth-focused care model and oversee a multidisciplinary team in community and short-term residential settings. You will manage program operations, staff, referrals, and outcomes from on-site offices, reporting to the Senior Director of Residential Services.

The role requires a Master’s degree in a helping profession, a license, and at least two years of related experience.

Qualifications

• A Master’s degree and license in a helping profession (Social Work, Mental Health Counseling, MFT, Creative Arts Therapy, etc.).
• At least two years of progressively responsible related experience.

Responsibilities

• Lead recruiting, hiring and onboarding of team members.
• Provide overall supervision to the team and monitor caseloads.
• Coordinate program intakes and discharges and supervise staff.
• Oversee short-term eight bed residential unit with 24/7 care.
• Develop training and on-call coverage schedules; be SME for the program.
• Engage with hospitals, schools, and providers for referrals and partnerships.
• Ensure fidelity to the program model and participate in required trainings.

$7,500 Retention Bonus!As a Director of Community Transition Time Program (CTTP), you will oversee a new model of care for youth to keep them out of unnecessary hospital settings. You will manage a multi-disciplinary team that provides services and support to adolescent youth and their families in the community and short-term residential settings. Responsibilities will include the management of the program, staff matters, client referrals, client treatment, financial viability, marketing of the program, interfacing with referral sources including local hospitals and county mental health providers, program manual, and program systems.
