Job summary

Advocate Health in Milwaukee, WI is seeking a Registered Dietitian to deliver comprehensive medical nutrition therapy for LVAD and heart transplant candidates and recipients across inpatient and outpatient settings. You will provide pre-, peri-, and post-transplant nutrition care, participate in transplant meetings, and support non-transplant heart failure nutrition therapy.

• Must be a Registered Dietitian (RD) with current licensure.
• Experience in hospital-based medical nutrition therapy (MNT) preferred.

Responsibilities

• Provide daily MNT care for inpatient and outpatient populations.
• Prescribe and assess enteral/parenteral nutrition support as needed.
• Lead nutrition-focused assessments and coordinate with interdisciplinary teams.
• Educate patients and families on nutrition plans and disease management.
• Document care plans and nutrition interventions in the EMR.
• Support transplant-related nutrition requirements and post-discharge planning.
