Job Summary & Responsibilities
As a Clinical Dietitian PRN you will identify patients at nutritional risk through the screening process. Provide nutrition assessment, care planning and diet/nutrition education appropriate for age, disease state and nutrition problem. Document nutrition care in the patient’s medical record. Provide interdisciplinary and collaborative care through day-to-day communication and interaction with the health care team. Contribute to nutrition care team knowledge and evidence-based practice. Provide education and training assigned. Participate in quality assignments.
- Identifies patients at nutritional risk via screening process.
- Provides nutrition assessment, diet/nutrition education appropriate for age, disease state throughout the hospital including specialized areas.
- Completes and documents nutrition diagnosis for all areas using the Nutrition Care Process.
- Uses evidence-based practice guidelines and clinical judgment in formulation of all nutrition care plans.
- Monitors and evaluates individual patient nutrition outcomes related to nutrition diagnosis, goals, and interventions in all areas.
- Participates as an integral member of the multidisciplinary team in hospitalist, adult/pediatric/neonatal ICU rounds prepared to communicate nutrition expertise with all appropriate healthcare providers.
- Performs nutrition-focused physical exams when warranted.
- Writes neonate formula recipes, parenteral nutrition, and enteral nutrition orders per consult or protocol.
- Acts as preceptor for dietetic interns. Complies with The Joint Commission and other regulatory agency standards.
Qualifications
- (RD/RDN) Registered Dietitian/Nutritionist
- (RD-STATE) Registered Dietitian - State
- Bachelor’s degree
- 1+ years of experience in a healthcare setting (preferred)
