You will collaborate with the healthcare team, develop care plans, and participate in health fairs and public education activities. RD credentialing timelines apply and BLS is required within 90 days of hire.

Qualifications

• BLS is required to be obtained within 90 days of hire.
• Registered Dietitian (RD) must be obtained within 6 months of hire. Verification statements required for those candidates awaiting RD credentialing.
• If Bachelor's Degree is in major other than Dietetics, must complete an ACEND accredited Didactic Program in Dietetics.

Responsibilities

• Assess and nutritionally manage acute and chronic disease in outpatient settings.
• Provide individual and group counseling, culinary nutrition, and community outreach.
• Develop and implement comprehensive care plans based on assessment and evaluation of patient needs.
• Communicate and collaborate with providers and other healthcare team members for referrals and care coordination.
• Serve as Sentara representative for health fairs, media outreach and community presentations.
