Job summary

Community Health Centers of the Central Coast is seeking a Registered Dietitian to coordinate nutrition care plans across multiple settings under supervision.

The role requires a Bachelor’s in Nutrition, active RD status, and the ability to communicate in English and Spanish; HIPAA and clinical nutrition experience preferred. We offer a patient-focused team and a supportive work environment in Nipomo, CA.

Qualifications

• Bachelor’s degree in Nutrition required.
• Active RD status required (or within 90 days).
• Diabetic Educator certification preferred.

Responsibilities

• Conduct a nutrition assessment and obtain nutrition history from patients.
• Consult with providers to determine patient’s nutritional needs and diet restrictions.
• Formulate menus for therapeutic diets based on medical condition and integrate with institutional menus.
• Develop and implement care plans based on nutritional needs and coordinate with other healthcare staff.
• Instruct patients about meal plans and dietary changes; set self-management goals.
• Evaluate nutritional care and provide follow-up continuity of care.
• Document nutrition care plans in patient records.
• Instruct patients and families in nutrition principles and dietary plans.
• Teach nutrition and diet therapy to other staff.
• Maintain HIPAA and confidentiality.
• Demonstrate knowledge of age-appropriate care.
• Understand protocols for domestic violence and abuse.
