Community Health Trust of Pajaro Valley is seeking a Registered Dietitian to independently apply nutrition interventions and lifestyle changes. You will plan, develop, and direct nutritional care, while coordinating with clinicians to support patient outcomes.

The role requires an RD license with ongoing education, strong communication skills, and ability to work in a team. Bilingual English/Spanish is preferred, with opportunities in diabetes education and community outreach.

Formación

• Possess the license to practice as a Registered Dietitian (RD) with ongoing education.
• Bilingual English and Spanish preferred.
• CDCES credential preferred.
• Ability to work in a team to ensure quality patient care.
• Strong problem-solving and time-management skills.
• Excellent written and spoken communication; attention to detail.

Responsabilidades

• Evaluate clients' health management programs and help plan future wellness.
• Perform in-depth nutrition assessments using labs, medical history, and diet factors.
• Prepare clinical program reports for ADA/ADCES accreditation.
• Develop and monitor EMR-based care plans with HIPAA compliance.
• Communicate with referring physicians, patients, and staff.
• Create culturally relevant education materials for clients and families.
• Educate on diabetes management, self-monitoring, and medication guidance.
• Coordinate with community resources to support patient care.
• Use motivational interviewing to help clients set self-care goals.
• Assist in developing new initiatives and education materials.
• Participate in community outreach and health promotion activities.
• Other duties as assigned.
