Job summary

The role requires a bachelor’s degree in nutrition or dietetics, CDR exam, and at least one year of experience with elderly populations; strong computer literacy and excellent communication are essential.

Qualifications

• Bachelor’s degree in food, nutrition, or dietetics from an accredited institution.
• Passed the Commission on Dietetic Registration (CDR) exam.
• Minimum of one year of experience working with the elderly population.
• Reliable transportation, valid driver’s license, and required auto insurance.
• Medical clearance for communicable diseases and up-to-date immunizations.
• Successful completion of a comprehensive background check, including verifications.
• Strong computer literacy, including Microsoft Office.
• Ability to effectively communicate in writing and verbally.

Responsibilities

• Provide high-quality nutritional care and actively contribute as a member of the team.
• Conduct initial and ongoing comprehensive nutritional assessments for all patients.
• Collaborate with the team to develop, implement, and revise individualized care plans.
• Offer nutritional counseling and education to patients, caregivers, and families.
• Facilitate nutrition-related interventions, including home-delivered meals and supplements.
• Monitor changes in weight, conditions, medications, and intake; report findings to the team.
• Guide team members and staff regarding patients’ nutritional needs and health issues.
• Incorporate cultural and ethnic dietary preferences into care planning.
• Participate in quality improvement initiatives and team meetings.
