Job summary

LiveWell Homecare Agency, LLC is seeking a registered dietitian to assess patients’ nutritional status, develop individualized care plans, and provide ongoing dietary guidance across in-patient and out-patient settings.

The ideal candidate holds a Bachelor’s degree (Master’s preferred for some certifications), RD credential, completed dietetic internship, and has strong organizational and communication skills to collaborate with clinical teams and educate patients.

Qualifications

• Bachelor’s degree, RD credential, and completion of a dietetic internship are required; Master’s is preferred for certain certifications.
• Strong organizational, critical thinking, communication, and interpersonal skills are essential.
• A minimum of three years of preferred technical and leadership experience is desirable.

Responsibilities

• Coordinate with clinical staff to create and document nutritional plans, identify potential issues, communicate patient needs to the healthcare team, ensure compliance with policies, and deliver counseling and health education.
• Provide nutritional care for inpatients and outpatients and monitor patient progress over time.
