This position's primary role is to provide Medical Nutrition Therapy (MNT) and Diabetes Self-Management Training (DSMT) to adults in the outpatient setting. Provides 1:1 and class education. Assesses nutrition status of assigned patients and implements all components of the nutrition care process, according to food and nutrition department policies and local regulatory standards (IDPH, TJC, CMS, etc.). Monitors and evaluates nutrition interventions, makes appropriate adjustments to plans, as needed, and reports results of quality outcomes in the inpatient and outpatient setting. Has specialty practice experience and working knowledge in specific nutrition discipline. This position is a part of the Clinical Career Ladder Program.
Pay Range: $53,144.00 - $82,368 Annually. Actual pay may vary based on experience, qualifications, education, location, licensure, certification, and peer comparisons.
