Job summary

Community Health Centers of the Central Coast, Inc. seeks a Registered Dietitian to coordinate nutrition care plans, assess patients, educate, and counsel across settings under the Health Education team. Maintain professional, punctual, and collaborative care.

Qualifications

• Bachelor’s degree in Nutrition or related field required; RD status within 90 days of employment.
• Bilingual in English and Spanish preferred; ability to read/interpret safety rules and write reports.
• Experience with word processing, spreadsheets, email, and EHR systems.

Responsibilities

• Conduct nutrition assessments and obtain nutrition history.
• Consult with providers to determine nutritional needs and restrictions.
• Formulate therapeutic menus and integrate with institutional menus.
• Develop and implement care plans based on nutrition needs.
• Educate on meal plans and set self-management goals.
• Evaluate care and provide follow-up.
• Document nutrition plans in medical records.
• Instruct patients and families in nutrition principles and diet plans.
• Teach nutrition to other staff.
• Adhere to safety policies and HIPAA.
