Job summary

SSM Health in Wisconsin seeks a registered dietitian to provide comprehensive nutrition assessments and care planning for patients and residents, evaluating treatment effectiveness and coordinating with the care team to ensure nutritional needs are met.

Responsibilities include education, discharge planning, and collaboration with physicians, nurses and therapists, while staying current with nutrition science and best practices. Community nutrition education may also be involved.

Qualifications

• Bachelor's degree in Dietetics or related field.
• Licensed Dietitian Nutritionist and RD/RDN certification per state requirements (Illinois, Missouri, Oklahoma, Wisconsin).
• No experience required.

Responsibilities

• Directs the nutritional care of patients and/or residents with special needs, including screening, assessment, intervention, care and discharge planning to meet nutrition needs and satisfaction.
• Adheres to federal, state and Joint Commission standards for nutrition care; monitors progress and adjusts plans; participates in discharge planning and education.
• Collaborates with physicians, nurses, therapists and other care providers; participates in care coordination meetings.
• Assesses educational needs, develops and delivers nutrition education materials and programs to patients and families.
• Performs ongoing education to stay current with advances in nutrition and diet therapy; contributes to guidelines and education materials.
