Employment Qualifications

Must be certified as a Registered Dietitian by the American Dietetic Association. Bachelor's degree in foods and nutrition and one to two years of supervised practice experience in foods and nutrition. Previous experience in health promotion and/or cardiac rehabilitation programs. Must demonstrate appropriate knowledge of the principles of growth and development from adult through the geriatric stage of the life span. Certified in Basic Life Support.

Essential Job Functions
  1. Develop and implement nutritional components, including curriculum, of programs offered through the Cardiac Wellness Center and Heart Vascular Institute, routinely evaluating and modifying content as needed. Ensure the nutritional content utilizes evidence-based information, meets program objectives, and is appropriate for the specific population.
  2. Access the online scheduler daily to schedule, cancel, reschedule, and block appointments as needed.
  3. Provide one-on-one nutrition consultations, document visits in medical records, and notify referring physicians.
  4. Assess patient nutritional status using established standards, determine age-specific and disease-specific requirements, and provide necessary care.
  5. Use effective interviewing methods to collect and validate data, review medical records, and discuss patient conditions with medical staff.
  6. Reference the online Nutrition Care Manual as needed.
  7. Collaborate with patients to set nutrition goals, ensuring they are evidence-based, planned, and measurable.
  8. Provide tailored nutrition goals, plans, and educational materials considering age, disease, cultural needs, and learning styles.
  9. Evaluate and chart patient progress, response to therapy, and educational outcomes at least once every 30 days and at discharge.
  10. Offer verbal and written feedback on progress and update nutrition prescriptions and weight goals accordingly.
  11. Identify patients needing follow-up nutrition counseling and coordinate care accordingly.
  12. Communicate with the team regarding patient difficulties and document in progress notes.
  13. Attend team meetings and participate in program development and community education programs.
  14. Familiarity with blood pressure cuffs and telemetry monitors, assisting as needed, and recording vital signs if required.
Other Job Functions
  1. Schedule patient visits for nutrition, education, or exercise and enter charges for services.
  2. Participate in quality assurance and utilization review of programs.
  3. Use computer programs like Excel and Access to track participation and outcomes.
  4. Monitor refrigerator temperatures and assist in daily clinic operations, including scheduling and communication with healthcare providers.
Working Conditions

The position is a Monday-Friday day shift. Evening hours may be required. There is a risk of exposure to communicable diseases, requiring universal precautions and protective equipment.
