Job summary

FutureCare Clinical Dietitians provide personalized nutrition care to residents through timely assessments and interventions. You will collaborate with the Interdisciplinary Team and apply the Nutrition Care Process in a skilled nursing environment.

A Maryland RD license and a degree in Nutrition or Dietetics are required, with 2+ years in the field preferred. We offer a competitive salary range and a $5,000 sign-on bonus, plus benefits, paid time off, flex/advance pay, EAP, and 401(k) matching.

Qualifications

• Maryland RD license required.
• Bachelor’s degree in Food and Nutrition, Dietetics, or related field.
• At least 2 years of Dietitian experience; skilled nursing facility experience preferred.
• Knowledge of Nutrition Care Process and applying all steps.

Responsibilities

• Complete timely nutrition and MDS assessments as required.
• Plan, develop, implement and monitor individualized nutrition care plans.
• Provide nutrition recommendations for patients on enteral nutrition support.
• Educate patients and families on nutrition.
• Apply current standards and evidence-based practices.
• Prioritize patient-centered care and residents’ rights.
• Perform monthly quality assurance and process improvement audits.

• Assess and identify the nutritional status of residents using validated screening tools.
• Complete timely nutrition assessments and Minimum Data Set (MDS) assessments as required.
• Plan, develop, implement and monitor the nutrition plan of care for each resident.
• Provide nutrition recommendations for patients reliant on enteral nutrition support.
• Provide nutrition education to patients and families.
• Utilize current standards and evidence-based practices.
• Prioritize patient-based care and residents’ rights.
• Complete monthly quality assurance and process improvement audits.

Qualifications

• Current Registered Dietitian license in the state of Maryland.
• Bachelor's degree with a major in Food and Nutrition, Dietetics, or a related field.
• At least 2 years of experience as a Dietitian, preferred.
• Previous experience working in a skilled nursing facility is highly desired.
• Knowledge of the Nutrition Care Process and ability to apply all steps of the process.
• A strong desire to deliver care in an empathetic, compassionate way.
• Superb organizational and time management skills.
• Decision making, problem solving, and collaboration skills are a plus.
• Excellent interpersonal, written, and verbal communication skills.
• Strong leadership skills and ability to work collaboratively with team members.
