Job summary

Apex Home Health, Hospice & Home Care in Utah is seeking a registered dietitian to provide medical nutrition therapy through the nutrition care process, assess patients, interpret labs, and tailor interventions across the care continuum.

Qualifications

  • Master's degree in a relevant field from an accredited program.
  • Registered Dietitian with the Commission on Dietetic Registration or eligible, with certification within 6 months if applicable.
  • Completion of an ACEND-accredited dietetics program and state licensure or certification.
  • Preferred experience in clinical dietetics and managing diverse patient populations.
  • Skills include medical nutrition therapy, physical assessment, nutrition diagnosis, education, critical thinking, customer service, communication, and computer proficiency.

Role Overview

This position is responsible for providing medical nutrition therapy through the nutrition care process to support individuals with common diseases and promote wellness. The role involves conducting nutrition assessments, interpreting lab values, identifying nutrition diagnoses, implementing tailored interventions, and monitoring outcomes. Collaboration with healthcare providers and referral to specialty dietitians are key duties. The role supports nutrition care across the healthcare continuum and participates in departmental improvement projects, with order writing privileges for registered dietitians.

Qualifications & Skills
  • Registered Dietitian with the Commission on Dietetic Registration or eligible, with successful certification within 6 months if applicable.
